What is MAVI?
MAVI is the only independent living center in Puerto Rico headquartered in San Juan and a satellite office in Ponce (South). We promote the independent living philosophy through peer counseling, advocacy, information and referral and skills development. Other services provided include: adaptive housing, accessible transportation, personal assistance services, psychology, assistive equipment and supported employment. We promote client control and productivity and their integration and inclusion in all aspects of our society.

What is Mental Health Association?
The Mental Health Association, Cortland County, provides the following: Family Support Services, Reach-Basics bimonthly lectures on or about mental health and three newsletters. We fulfill these activities through the labor and activities of a mental consumer work force. Current employees are responsible for all secretarial work, printing and copying, collating, envelope stuffing, preparations and reports for US Postal Bulk Mailing regulations, cleaning, composing news articles, maintaining work force statistics etc. Each employee is assigned to his/her own work area which includes a desk, chair etc.A newly established work program is our Courier/Messenger service which we plan to enlarge and one day provide this service to local businesses.

What is the Mental Health Association of Erie County, Inc.?
The Mental Health Association of Erie County, Inc. is a private non-profit corporation whose core mission is to improve care and treatment for persons who may have a mental illness and to promote mental health through education, advocacy, public information and prevention activities. Client Advocacy Service is a department of the Mental Health Assn. of Erie Co., Inc. which provides civil legal services at no charge to mental health consumers in Erie and Niagara Counties. Most of the legal advocacy involves public benefits including supplemental security income, social security disability benefits, public assistance, Medicare and Medicaid. CAS also provides training on legal issues to professionals, consumers and family members.

What is the Mental Health Association of Essex County?
The Mental Health Association in Essex County, Inc. is a not-for-profit human services organization chartered to: increase community awareness of mental illness, promote improved functioning for people with mental illness, and provide advocacy, referral, and preventive services.
The programs of the Mental Health Association include:
- Supported Housing: This program enables people to locate and secure housing in their own communities. Assistance with the costs of renting provided to those who qualify.
- Assisted Competitive Employment and Supported Employment: MHA staff is in place to help people determine the career, which makes the most sense. We also provide assistance on developing the tools for the work, finding jobs and retaining employment.
- Intensive Case Management: Each individual in this program receives several home visits every month. This is intended to ease the sometimes-difficult task of community living and to assist with the achievement of success and fulfillment.
- Community Case Management: Our case managers provide support, education, training and information to assist people who wish to live independently within their communities. The case mangers make visits to people in their own homes.
- Community Centers: MHA has sites in local communities where people can go in order to establish friendships and support. The centers are located in Westport, Ticonderoga, Port Henry, Elizabethtown, and Lake Placid.
- Crisis Alternatives Program: This program is new to MHA in late 1998. Its goal is to provide support and assistance to people who are feeling the most intense anxieties. Staff works in the evenings and on weekends to provide telephone counseling, home visits, and respite apartments to those to those in need.
- Self Help: MHA serves as a conduit for funding for consumers of mental health services involved in self-advocacy, education, and empowerment.
- Education Assistance: Many people in our community have not achieved the lower level of education they wish for themselves. This program seeks to reduce barriers for people who agree to actively pursue their GED, Associates, Bachelors, Graduate Degrees, or other academic endeavors.

What is the Peer Advocacy Program?
Peer Advocacy Program, which outreaches to recipients of mental health services and their families, recruits and trains people to participate in the mental health services planning process, identifies educational needs and has established an advocacy informational center and network. What is the Mental Health Association in New York State, Inc.?
The Mental Health Association in Ulster County, Inc., (MHA) was chartered as a not-for-profit organization in 1959 by a group of physicians and concerned individuals to develop a coordinated citizen's movement to promote mental health and to work toward improved care and treatment for persons with mental illnesses. The mission of the MHA is to create communities that respect diversity and are dedicated to the full participation of all persons.
To achieve its mission, the MHA sponsors a variety of educational, advocacy and service programs, including mental health education, rehabilitation, residential services, family and youth support services and therapeutic foster care.
The following is a list of our programs offered:
- Client Services Mental Health- GOAL: To provide opportunities for persons recovering from mental illnesses and individuals with developmental disabilities to participate in recreational, educational and leisure activities that enhance their socialization skills and community integration.
- Education and Advocacy Services- GOAL: To develop educational programs which meet the needs of Ulster County residents through education, training and advocacy services.
- Residential Services- GOAL: To provide a variety of community-based housing alternatives to enable persons recovering from mental illnesses to become and to remain a part of the community.
- Family-Based Treatment Services- GOAL: To maintain/provide a family environment for children and youth with special emotional needs, who might otherwise be placed in a more restrictive setting.
- Family and Youth Services- GOAL: To provide programs that enhance family life by enabling the development of healthy relationships between parents and children and promote the well-being of all children.

What is the Mental Health Resource Center?
The Mental Heath Resource Center provides outpatient treatment for children and adults with mental, emotional and behavioral problems. Our professional staff provides a variety of therapeutic methods, including: individual, family, group, couple, and play therapies; stress management; follow-up to psychiatric hospitalization; and, medication monitoring and administration. The Center also provides group therapy for mentally ill chemical abusers (MICA). As the convener of the Youth Incentive Program for Essex County, the Center plans for youth in danger of residential placement or those youth returning form residential placement to the community.

The Mercer County Office for the Disabled provides comprehensive services for people who are developmentally delayed or disabled. What is the Metropolitan Hospital Center/ CSS Psychiatric Day Treatment Center?
Located in the East Harlem section of Manhattan, the CSS Psychiatric day Treatment Program at Metropolitan Hospital center provides full-time continuing drug treatment for severely and persistently psychiatric clients. We serve sixty clients and have a multidisciplinary staff composed of a psychiatrist, psychologist, nurse clinician, two social workers, two rehabilitation counselors, two activity therapists and one recreational therapist. All staff are professionals. We have art therapy rehabilitation students and peer specialist interns who work part-time at internships at our program. We have a large Hispanic population and have four bilingual staff members to serve them. We provide case management, psychopharmacology, health screening, assistance with benefits and housing applications, individual and group counseling, supportive and cognitive therapy, educational vocational rehabilitation assessments, counseling and referrals. There is no time limit. Clients are expected to attend Monday through Friday from 9:00am to 3:00pm. The program offers a milieu where clients have daily structure, support and opportunities to socialize and recover. Many of our clients are referred from inpatient psychiatric hospital units and outpatient psychiatric clinics and housing programs. While at CSS clients have opportunities to volunteer to work in the program running self-help groups and be involved in counselor-run businesses and vocational activities.

What is MAVI: Caribbean Benefit Planning Project?
Movimieto para el Alcance de Vida Independiate or MAVI is
the only Center for Independent Living in Puerto Rico.
We are a not-for-profit organization serving people with disabilities
incorporated under the Commonwealth of Puerto Rico. Our
mission is to promote the independent living philosophy and
consumer control in all aspects of his/her life. We advocate
for the inclusion, integration and productivity of people with
disabilities in all aspects of society.
We partner with
the Virgin Island Association for Independent Living to create
the Caribbean Benefit Planning Project to serve 20,000 beneficiaries
with disabilities in this area. The project will provide
the benefit planning, assistance and outreach in this region
through the creation of a 1-800 line, a webpage, development
of educational and training opportunities and networking.
There will be case management services to those individuals
who desire to develop an individual work plan.
Among
other limited services that we provide include: personal
assistance services, adaptive housing, psychology and transition.
We have a main office in San Juan and a satellite office in
Ponce.

What is the Muscular Dystrophy Association?
Since its founding in 1950, the Muscular Dystrophy Association has become on of the nation's largest and best known voluntary health agencies. MDA provides comprehensive medical services to tens of thousands of people with neuromuscular diseases at more than 200 hospital-affiliated clinics across the country. The Association's worldwide research program, which funds some 400 individual scientific investigations annually, represents the largest single effort to advance knowledge of neuromuscular diseases and to find cures and treatments for them. At facilities in MDA's nationwide network of hospital-affiliated clinics, patients receive diagnosis and follow up care from specialists in neuromuscular disease. In conjunction with this medical care, MDA-through its some 150 chapters in the U.S. and Puerto Rico- provides many other direct services, including assistance with purchase and repair of wheelchairs, recreation at MDA summer camps, and selected transportation services.

What NAMI - Familya?
NAMI - FAMILYA of Rockland County serves families of people with mental illness through support, education, and advocacy. We provide a wide variety of support services, informational programs and materials for family members, mental health professionals, and consumers of mental health services, as well as the general public.
Founded in 1981 by a small group of families with relatives with mental illness, the organization has grown steadily in number and influence. NAMI - FAMILYA is the Rockland County affiliate of NAMI-NYS and NAMI (the National Alliance for the Mentally Ill.) In concert with thousands of similar grassroots organizations throughout NYS and the U.S., we are recognized as a significant advocate in defending the rights and advancing the interests of individuals with mental illness.
The organization is committed to improving the lives of people afflicted with mental illness, offering hope and assistance to their families, advancing the search for cures, fostering understanding and combating discrimination and stigma through public education. Dedicated volunteers carry out our activities and programs.
Regular meetings are held on the first and third Wednesday of each month in room 119, Building F of the Dr. Robert L. Yeager Health Center, Sanatorium Road, Pomona, New York. Our RAP sessions provide emotional support to family members, as well as information and guidance regarding coping techniques and available resources. Our educational programs on the third Wednesday of each month feature experts on a variety of relevant topics dealing with such issues as research, treatment and rehabilitation, services and entitlements, etc. Our monthly newsletters are as on-going source of information and education. We periodically offer 10-week Family-to-Family Supportive Training course to interested family members at no charge. This series is invaluable to people whose loved ones have recently been diagnosed with mental illness.
We also engage in many educational outreach projects to the community at large. These include annual presentations in health, sociology, and psychology classes at several local high schools colleges, public forums on a variety of topics imparting on people with mental illness, and dissemination of brochures and information about mental illness. We provide in-service sensitivity training, both for Rockland County Department of Mental Health and the Rockland Psychiatric Center.

What is the Nassau County Office for the Physically Challenged?
The Nassau County Office for the Physically Challenged serves as the primary disability rights, advocacy, information and referral, educational, and constituent services entity on behalf of persons with disabilities in this county of 1.3 million people. We estimate, based upon US Census Bureau data, that we serve some 200,000youngsters and adults with disabilities in this county alone, and millions more nationally through our legislative out reach and partnerships with disability rights groups nation-wide.

What is the National Alliance for the Mentally ILL of New York City?
The National Alliance for the Mentally ILL of New York City, Inc. (NAMI-NYC Metro) is dedicated to improving the lives of people with neurobiological disorders (mental illness) and their families and friends through support, education, advocacy, and research. Founded in 1979, NAMI-NYC Metro is a grassroots, non-profit, self-help, organization of family members and friends of the mentally ill, as well as people who suffer from mental illness themselves ("consumers"). NAMI-NYC Metro is one of the largest chapters of the National Alliance for the Mentally ILL (NAMI) and the New York Alliance for the Mentally ILL (NAMI-NYC)
NAMI-NYC Metro offers practical support, useful education, comfort and understanding to those in the greater New York City area who suffer or have family members suffering from neurological disorders: primarily schizophrenia; bipolar disorder (manic-depression); depression; panic and anxiety disorders, including obsessive-compulsive disorders; multiple diagnosis, including mentally ill, chemically addicted (MICA) consumers; and borderline personality disorders.
NAMI-NYC Metro works with the media to increase public awareness and to advocate for improved services for the mentally ill. NAMI-NYC Metro is waging a campaign against stigma and discrimination of the mentally ill; it advocates for insurance parity for mental illness; and it is a major advocate for increased and improved treatments and services for those who suffer from mental illness and their families.

What is the National Association for Visually Handicapped?
NAVH is the only voluntary health agency which serves the "hard of seeing" exclusively--not totally blind, for whom over 800 helping agencies exact. Our services include visual aids counseling and distribution, large informational materials and a free-by-mail loan library of over 7,000 titles, educational outreach to lay persons and professionals, emotional support on individual and group levels, advocacy, and referral services. No person is refused services because of inability to pay. Our program remains the only one of its kind for over 47 years.

What is the National Down Syndrome Society?
The National Down Syndrome Society was established in 1979 to ensure that all people with Down syndrome have the opportunity to achieve their full potential in community life. Since that time, this not-for-profit organization has become the largest non-governmental supporter of Down syndrome research in the United States. Today, NDSS continues to increase public awareness about Down syndrome and discover its underlying causes through research, education and advocacy.

What is New Alternatives for Children, Inc.?
Founded in 1982, New Alternatives for Children (NAC) is the only agency in metropolitan New York whose exclusive mission is to provide child welfare services for severely disabled and/or chronically ill children and their families. NAC's mission is to ensure that medically fragile children are not hospitalized beyond medical need, to open a world of opportunities to them, to give them a permanent home and family in the community and to support the whole family in reaching their potential. NAC's innovative services achieve permanency through timely adoptions or family reunifications. No children are allowed to languish in foster care and none has ever returned to live in a hospital. During the course of NAC's 18-year history, over 1,370 children have been served.
Annually NAC now reaches more than 350 children with disabilities and their siblings and families through one of four programs: Preventive Services, Foster Care & Adoption, PIP (Partners In Parenting, a prevention after-care program), and PLAN (Post Legal Adoption Network). Services in all four programs include: comprehensive social services, medical and mental health care, educational services, recreation, transportation and auxiliary services.

What is the New Jersey Commission for the Blind and Visually Impaired?
The New Jersey Commission for the Blind and Visually Impaired is the designated state agency that provides services to New Jersey citizens who are blind and visually impaired. Vocational Rehabilitation services are provided pursuant to Title IV of the Workforce Investment Act of 1998. In addition to vocational rehabilitation services, the Commission provides services to eligible pre-school and school aged children through the educational services program and provides independent living services through the Adaptive Living Skills and Eye Health Services programs. There is also a Prevention Services Unit in the Commission. The Commission operates an Instructional Resource Center for individuals served through the educational services program and a comprehensive Rehabilitation Center for transition students and adults.

What is The New Jersey Developmental Disabilities Council?
The New Jersey Developmental Disabilities Council gives people with disabilities and their families a voice in public policies which affect them. It funds projects to advance policies the Council supports.
The Council has several other efforts in the areas of housing, employment and community capacity building. The Council is comprised of 32 public and private members appointed by the governor under terms of State legislation and federal mandate. The Council is the only independent advocacy agency in the State dedicated to advancing the independence and productivity of people with developmental disabilities.

Training provides matching grants to employers to upgrade worker
skills to enhance the creation and retention of high wage, high
skill jobs. The Rapid Response Team assists employers that are

What is the New Jersey Division of Disability Determination Services?
The New Jersey Division of Disability Determination Services processes and adjudicates claims for disability under Title II and Title XVI of the Social Security Act. Our mission is to render disability determinations decisions correctly, timely, and in a cost effective manner. To that end, we have four geographical claims regions to serve the State of New Jersey: North and Upper North in Newark, NJ, central in New Brunswick, NJ, and south in Trenton, NJ. The Division employs approximately 350 employees and makes decisions on about 80,000 claims per year.

The New Jersey Division of Vocational Rehabilitation Services (NJDVRS) is the state agency which provides services to enable persons with disabilities who are having problems keeping their jobs. What is the New Jersey Disability Determination Services?
The New Jersey Disability Determination Services (DDS) is a division of the department of Labor fully funded by the Federal Government, and is responsible for developing medical evidence and making the determination on whether residents of New Jersey are or are not disabled under Social Security disability law. While Social Security Administration handles the technical eligibility issues, a two-person adjudicative team consisting of a physician or psychologist and disability adjudicator makes the determination on medical / vocational / legal eligibility. After DDS makes the determination, it returns the case to the Social Security Administration for appropriate next action depending on whether the claim was allowed or denied. If the claimant was found disabled, SSA prepares necessary payment actions. If the claim is denied, SSA retains the file in case the claimant wants an appeal.

What is the New Jersey Developmental Disabilities Council?
The Developmental Disabilities Council gives people with disabilities and their families a voice in public policies that affect them. It funds these projects to advance the policies it supports:
Election Challenge Grants, in collaboration with the Council's Monday Morning Project, have registered 8000 new voters with developmental disabilities and assisted many of them to get to the polls on Election Day over the last 6 years, thus increasing their participation in the activities of their communities.
The Office of Disability Services, created in response to the Monday Morning Platform, now has Division status and its budget for personal assistance increased ten-fold through Medicaid Funds.
Over the last 4 years, the Council has recognized personnel in 51 school districts with Exemplary Practice in Inclusive Education awards for developing innovative ways to support students with significant disabilities in the regular classroom. Winners receive a small stipend to defray the costs of sharing their expertise with other school districts throughout the state.
The People Support Project assists 344 people living in developmental centers who are moving to the community as a result of the Olmstead decision to prepare for their new lives. First of its kind in the nation, its provides training in individualized planning, exposure to community living options, group and individual supports and advocacy services.

What is the NJ Division of Disability Services?
The NJ Division of Disability Services is a part of the Department of Human Services. It offers a "Single point of entry" for individuals with disabilities (regardless of age or nature of disability) into the human services system.

What is the NJ Division of Mental Health Services/Supported Employment Programs for Persons with Psychiatric Disabilities?
The NJ Division of Mental Health Services is the state mental health authority in New Jersey. Its mission is to promote opportunities for persons with serious mental illness to maximize their ability to live, learn, work and socialize in communities of their choice. This is accomplished through a comprehensive system of care including inpatient psychiatric setting and community based support services. The Division operates four regional and three specialty hospitals. It purchases community mental health services through contacts with about 125 not for profit providers. Community services include emergency screenings, outpatient partial care, supported employment, residential, integrated case management, and programs for assertive community treatment and systems advocacy.

What is New Jersey Protection and Advocacy, Inc.?
New Jersey Protection and Advocacy, Inc. (NJP&A) is New Jersey's federally funded Protection and Advocacy system for persons with disabilities. NJP&A is a private non-profit organization established to advocate for and protect the legal, civil and human rights of citizens of New Jersey with disabilities.
The New Jersey Client Assistance Program (CAP) is a federally funded program that advocates for and protects the rights of individuals with disabilities who are seeking or receiving vocational rehabilitation services. CAP provides individual legal representation and non-legal individual and system advocacy. CAP also provides outreach and education. CAP usually works through negotiations, but is authorized to pursue legal, administrative and other appropriate remedies.
CAP is an integral part of New Jersey Protection and Advocacy, Inc. (NJP&A). CAP is funded by the U.S. Department of Education/Rehabilitation Service Administration.

The New Jersey State Office on Disability Services is located within the NJ Department of Human Services. What is Brooklyn Occupational Training Center (NYC Board of Education)?
This mission of the Brooklyn Occupational Training Center (BOTC) is to have young adults with disabilities play an active role in planning for their future by realizing their talents, strengths and capabilities. The program at the BOTC helps students make informed choices about their futures by facilitating their acquisition of specific academic and work skills, appropriate work habits and supports necessary to ensure community inclusion.
The Brooklyn Occupational Training Center is a New York Board of Education, District 75, special education secondary school for developmentally disabled young adults. The BOTC provides these IEP diploma bound students with a well balanced program that stresses in-school and community-based/vocational experiences. The BOTC utilizes an "immersion" approach founded on the premise that everyone can and should be allowed the opportunity to be a constructive and contributing citizen regardless of their disability. This system helps identify and put into place the community and the world of work.
The Brooklyn Occupational Training Center is committed to effecting a permanent, positive change in the lives of young men and women. Linkages between the school and the business community are maintained to support community based learning experiences for students. These experiences serve as an introduction to employment for our students and familiarize potential employers with the students.
In the last year of school, the student, their family, the school, VESID (The Office of Vocational and Educational Services for Individuals with Disabilities) and an adult service provider work together so that paid employment can be secured. A person centered or life plan (PCP) is developed by the team to help determine which employment opportunities are viable considerations. This is based on the student's past vocation/work experience, hopes, dreams and special accommodations or supports. The school and adult service agency jointly implement the goals of the PCP and are responsible for the development of employment opportunities, job coaching, and other supports the student may need in order to make a smooth and successful transition from school to work.

What is NYC Department of Health Physically Handicapped Children's Program?
The Physically Handicapped Children's Program (PHCP) is a diagnostic and medical treatment and rehabilitation program administered by the NYC Department of Health, under guidelines developed by the NYS Department of Health. The goal of the program is to ensure that physically handicapped and chronically ill children minimize the disability and maximize rehabilitative services so that they can reach their full potential. PHCP provides a means of "last resort funding" limited to a maximum of $1500 per year of eligibility per child, and cannot substitute for or duplicate coverage by the family's private health insurance or any other third party payer. PHCP also provides care coordination services through its Children with Special Health Care Needs resource program. The purpose of this program is to help assure accessible, appropriate coordinated, culturally competent, comprehensive, community-based care for chronically ill and disabled children (ages 0-21 years of age) and their families. These programs do not perform clinical services.

New York Presbyterian Hospital, Westchester is a leading provider of behavioral health care, education and research. The hospital is a major division of New York Weill Cornell's Department of of Psychiatry. What is the New York State Commission for the Blind & Visually Handicapped?
The NYS Office of Children and Family Services' Commission for the Blind and Visually Handicapped (CBVH) provides vocational rehabilitation services to individuals who are legally blind and reside in New York State. Services are coordinated by counselors and children's consultants located in six regional offices. Our services include:

The State of New York Office of Children and Family Services, Commission for the Blind and Visually Handicapped (CBVH) provides rehabilitation services to individuals who are legally blind and reside in New York State. CBVH programs include:

Services are coordinated by counselors and children's consultants located in eight regional offices.
Who is eligible?
Any New York State resident who is legally blind may be eligible for some of the services provided by CBVH. If you are unsure if you are legally blind, CBVH can assist you in determining eligibility.
Employment Program
If you are interested in obtaining your first job, returning to work or keeping your job, you will be assisted by a vocational rehabilitation counselor. A plan of services, which can include training and/or education, will be developed by you and your counselor. You will have the opportunity to review and adjust the plan with your counselor on an ongoing basis. Upon successful completion of the plan requirements, CBVH will assist with job placement. Services are also available for those interested in learning skills for independent home management.

What is the New York State Commission for the Blind and Visually Handicapped?
The Hempstead Long Island Office of the New York State Commission for the Blind & Visually Handicapped (CBVH) is the Agency responsible for providing vocational rehabilitation services for legally blind residents of Nassau, Suffolk, & Queens counties. Our vocational Rehabilitation counselors meet with applicants to determine their eligibility for vocational rehabilitation services, develop an individual plan of employment (IPE), and implement that plan, we have a counselor who provides services to children ages birth to 14. We have a counselor who provides services to teens, and a staff orientation & mobility specialist.

What is New York State Commission on Quality of Care/ Client Assistance Program?
The Client Assistance Program (CAP) is a statewide network of advocates available to assist New Yorkers with disabilities in gaining access to VESID and CBVH services. CAP advocates provide information support and strategies for negotiating VESID, CBVH and related service systems. CAP advocates are independent of VESID or CBVH and are committed to promoting consumer access to quality services and supports.

What is the New York State Department of Labor/New York Works?
New York WORKS is a new initiative of the New York State Department of Labor to demonstrate how persons with disabilities succeed in employment when targeted resources are available to workers and employers. The main goals of New York WORKS are:
- To better focus the resources available to individuals with disabilities and employers directly related to workforce development;
- To advise individuals with disabilities on how to retain essential "safety net" benefits, such as health care coverage, as they enter the workforce;

What is The New York State Developmental Disabilities Planning Council?
The New York State Developmental Disabilities Planning Council (DDPC) is a planning and advocacy body that was established in 1971 under the auspice of Developmental Disabilities Assistance and Bill of Rights Act (DD Act). This Federal legislation created State DD Councils to work under the direction of the Governor and to promote advocacy, capacity building and systems change for persons with developmental disabilities in each State. The DDPC is comprised of volunteer members, of whom at least 60% must be persons with developmental disabilities or their family members/guardians.
The DDPC assists State and local government, public and private agencies, the business community, and people with disabilities and their families in developing opportunities for community living and participation. This type of assistance enables people with developmental disabilities to achieve their maximum potential through increased independence, productivity, integration and inclusion in their communities. To accomplish this, the DDPC:
- Establishes demonstration grants and projects designed to create and implement innovative approaches for persons with developmental disabilities to meet their needs, increase their self-determination and community inclusion;
- Provides comprehensive training and technical assistance for constituents, families, professionals, and key community partners;
- Assists individuals and their families to access necessary services and supports; and
- Works with key community partners to help shape policies, procedures and approaches that are more responsive to the needs of persons with developmental disabilities and their families.
To the greatest extent possible, DDPC projects and initiatives are developed in direct response to the concerns and ideas voiced by consumers, families, service providers, policymakers and other key stakeholders. Through these initiatives, the DDPC promotes the capabilities of children and adults with developmental disabilities so that they will have the opportunity, to the greatest extent possible, to make their own choices and decisions in life.

What is NYSILC?
The New York State Independent Living Council, Inc. is made up of representatives, a majority of whom has disabilities, from around New York State, who are appointed by the Board of Regents. What is The New York State Rehabilitation Association/ Rehabilitation Research and Training Institute?
The New York State Rehabilitation Association is a statewide not-for-profit organization with a growing membership of over 140 member agencies. NYSRA is a leader in advancing public policy that benefits people with disabilities and is a source of information, education and networking opportunities for organizations and employees who provide these services. NYSRA's services include: regulatory and legislative advocacy, technical assistance, information sharing and the development and professional growth of rehabilitation agency personnel.
In 1991, the Rehabilitation Research and Training Institute (RRTI) was formed as a separate corporation. RRTI recognizes that skilled rehabilitation agency staff and management are critical to the most effective delivery of such programs. Seminars cover all areas of rehabilitation services with particular focus on employment preparation for people with disabilities. RRTI training programs are offered at a substantial savings to NYSRA member agencies.

What is NYS VESID/Vocational Rehabilitation?
Provides diagnostic evaluation services for such as vocational testing and medical examinations; vocational counseling and training services; physical restoration services such as surgery, prosthetic appliances and physical therapy; job placement services; supportive services such as independent living services, house maker services and driver training. Special transportation for severely disabled clients and interpreters for the deaf are also available. Professional staff are available to meet with individuals in the community.
Eligibility Requirements:
To be eligible for VESID services you must have a physical, emotional or learning impairment that interferes with your ability to work. There must also be a reasonable chance that you will become gainfully employed if you receive rehabilitation services, and you must require vocational rehabilitation services.

What is VESID of Staten Island?

What is NOSSCR?
NOSSCR is a national association of attorneys whose practice includes representation of Social Security disability claimants. We have been in existence for twenty years, and have approximately 3,600 members. We believe that our monthly newsletter, bi-annual conferences, and our clearinghouse of information are important tools available to claimants' representatives to enhance their capability in assisting this group of claimants.
We have a toll-free telephone number (800-431-2804) on which claimants may call us and obtain, without obligation, the name of a private attorney in his or her area. Most of our members work on a contingent fee basis, and any fee must first be authorized by SSA.

What is Office for the Disabled?
The purpose of the Office for the Disabled is that of ombudsman with a mission to establish a viable advocacy program whose objective is to gain equality, opportunity and security for all people with disabilities through education, outreach and legislation. What is Office for the Disabled?
Since its inception in 1975, the Office has functioned as a clearinghouse of information for people with disabilities, their families and advocates. Each month we handle approximately 1000 phone calls from people who are looking for transportation, housing, removing architectural and communication barriers, and employment. We also provide information about the Americans with Disabilities Act and other laws that guarantee people with disabilities basic civil rights. These are a broad range of issues that are critical to the opening process of bringing disabled people into the "mainstream" of society. Our goal has always been to be the last call-or, at least the next to last call-anyone has to make for the answers they seek.
Apart from information and referral for the public, we also consult with other County Department's to ensure that Westchester County's policies, programs and facilities are in compliance with the mandates of the Americans with Disabilities Act (ADA) of 1990.

What is Options for Independent Living?
Options for Independence is an Independent Living Center which works with all persons with disabilities, their families and friends. The center provides individuals with Benefits Advisement, Educational Advocacy and Service Coordination. The Access Advocate operates two ramp programs in the vicinity of Cayuga County (one in the City of Auburn and one in the County of Cayuga). The Peer Advocate works with individuals on budgeting, housing and employment issues. The Center also has Kids on the Block Available for public Performances for the community (both in Seneca County and Cayuga County). Kids on the Block is an Early Childhood program exposes children to disability awareness - it also enhances their world about the people who live around them in their community. The office also host a Traumatic Brain Support Group, with a consumer Facilitator and is a host of Low Vision Support Group. Options for Independence does a great a great deal of work in information and referral and serves on several community task groups and boards to discuss the issues specific to the disability community. A quarterly newsletter id distributed free of charge and an annual Spring Conference is hosted each year.

What is the Office of the Ombudsman for Persons with Disabilities?
The Office of the Ombudsman for Persons with Disabilities was created by the Puerto Rico Law No. 2 of September 27, 1985 and is the Agency designated by the Governor to be the protection and Advocacy System of Puerto Rico.
The mission of the Agency is to guarantee the protection of the rights of persons with disabilities, in order for them to achieve full integration and participation in all aspects of our society.

Parent to Parent of New York State is based on the philosophy that parents, who have themselves experienced the joys and frustrations of parenting a child with a disability, can offer new parents a valuable resource.

What is Pederson-Krag Center?

The Progressive Center for Independent Living?
The Progressive Center for Independent Living provides the resources for living life to the fullest. People with disabilities who live in Mercer and Hunterdon counties can obtain four services: information and referral, advocacy, peer support and independent living skills and training. What is the Project Independence of Queens?
Project Independence of Queens is a not-for-profit agency securing persons with developmental disabilities. Services are provided both to the individual and the family. What is Project Renewal?
Job Links' parent agency is Project Renewal, which has been providing New York's homeless with shelter, counseling, addiction-recovery services, and employment services for over 30 years.
Now in its third year, Job Links' is a supported employment program for adults with documented Axis I psychological diagnosis. These adults are referred by caseworkers in our or other programs, and they share one thing in common: they want to work. Job Links' goal is to get clients into part- or full-time competitive employment as quickly as possible. Our five-person staff works with clients through the entire job-seeking process, including intensive, non-time-limited retention services after employment is secured. Our job developer works with each client individually to find employment in fields as diverse as mailroom clerk and laboratory research assistant.
Mental health consumers interested in becoming part of Job Links should have their caseworker or medical professional contact the Job Links program coordinator to set up an initial intake screening.

What is the Office of Ombudsman for Persons with Disabilities?
The Office of Ombudsman for the Persons with Disabilities is the state agency designated by the government to be the protector and advocacy system in Puerto Rico.

What is the Puerto Rico State Rehabilitation Council?
The Council was created as a result of amendments to the 1992 Rehabilitation Act, which established that each state must have an advisory council or independent commission. The function of the Council is to review, analyze and advise the state unit regarding the performance of its functions and responsibilities.
The purpose of the Council is to incorporate the participation of citizens, which includes applicants, consumers, advocates, and other persons interested in the process carried out by the Vocational Rehabilitation Administration in providing services to individuals with disabilities, and to ensure the quality and excellence of the services, and that services be provided equitably.

What is Puerto Rico SILC?
The Puerto Rico Statewide Independent Living Council (SILC) was created in 1994 by disposition of the Rehabilitation Act of 1973, as amended. Its primary function is to promote a philosophy of independent living, including a philosophy of consumer control, peer support, self-help, self-determination, equal access, and individual and system advocacy, in order to maximize the leadership, empowerment independence and productivity of individuals with disabilities and the integration and full inclusion of individuals with disabilities into the mainstream of society.
To carry out its function, the PR SILC will prepare, in conjunction with the Vocational Rehabilitation Administration, a state plan for the provision of independent living services in the state and the development of centers for the provision of independent living services.
The PR SILC will promote educational activities designed to create awareness in the general public about the respect and dignity that people with disabilities deserve.

The organization provides disabled persons with work-site scenario job training and sustained employment. Queens Independent Living is not a place to live. Rehabilitation Management Group, Inc. (RMG) is owned by Roseann S. Falise, Ph.D., C.R.C., C.A.S., C.L.C.P. RMG is located in Central New York, within easy access to Watertown, Syracuse and Rochester.
Roseann Falise has assisted individuals with disabilities in successfully starting their own business, obtaining education or retraining, and finding new employment. She can also assist in developing a PASS plan. She has worked with individuals with catastrophic injuries and in a supported employment program for individuals with developmental disabilities or acquired brain injury. Dr. Falise was a vocational case manager for individuals who are injured, through NYS Auto and Workers' Compensation. She also is a Nationally Certified Rehabilitation Counselor and a Certified Life Care Planner.

What is the Resource Center for Accessible Living?
The Resource Center for Accessible Living is a small, not-for-profit agency, one of 35 ILC's which service people with disabilities and their families. Services include; benefits advisement and advocacy; special education advocacy and parent training; information referral; consultants and a loan closet for durable medical equipment.
RCAL has an ongoing "disability awareness" program which goes into area schools to advocate students about the accomplishments of people with disabilities and the potential for all people with disabilities to achieve and to raise students awareness of physical barriers or attitudes which still serve to slow the pace of full integration of people with disabilities into the community.
RCAL also provides case management and service coordination through the home and community-based services program for individuals with developmental disabilities and is also expanding outreach services to students transitioning out of high school.

What is Resources for Independent Living , Inc.?
Resources for Independent Living (RIL) is unique among Independent Living Centers in New Jersey. In addition to offering the four core services: information, referral, peer support, and advocacy; RIL has a "Get and Give" program. Every consumer is provided with high quality service by trained independent living specialists; then is provided an opportunity to give back to the community. RIL believes every consumer has skills, abilities and talents, which may have yet to be discovered and fostered. This philosophy enhances independence and even prepares consumers for the work world, if that is his/her goal.
At this point in time, RIL offers peer support groups, mobility classes for the blind, ASL classes, and basic computer keyboard training.
Outreach to youth includes presentations to regional Girl Scout Councils. Moving from module to module, the girls spend an entire morning/afternoon learning how to celebrate our differences. They learn the Girl Scout pledge in ASL, spend a virtual day with a blind person, and experience first-hand wheelchair mobility.
Transportation, housing, and links to community-based programs are an important part of the mission. RIL is the epitome of independent living for the disabled living in Burlington County, the largest county in NJ.

Resources for Children with Special Needs is a comprehensive, independent, not-for-profit information, referral, advocacy, training and support center for New York City parents and professionals looking for all kinds of programs and services for children from birth to 21 with learning, developmental, emotional or physical disabilities. What is Riverdale Mental Health Association?
Access to Competitive Employment (ACE) at the Riverdale Mental Health Association, provides individual job placement, job coaching, and follow along support for adults with serious and persistent mental illness. The goal of ACE is to help participants make a successful transition to the competitive labor market. Our staff assists at every stage in this process, from employment readiness, to instruction in job search skills, to work adjustment support and long-term coaching and advocacy. Contact is mentioned with each participant's mental health treatment provider. There is no waiting list. Generally, participation begins from 1-3 weeks after receipt of application. In developing jobs, we strive to find suitable, rewarding work and advancement opportunities that match participants' career and employment needs interests. ACE participants come from a wide range of age groups, education levels, and work backgrounds. Participants must be 18 years of age or older, in treatment for mental illness, able to travel to RMHA, and be eligible for VESID services.
The Riverdale Mental Health Association is a non-profit, independent agency established in 1959. Other RNHA services include clinic treatment, continuing day treatment, substance abuse, and a community residence.

What is the Rockland Psychiatric Center?
RPC services people 18 years of age and older with severe and complex mental illnesses who live in the Rockland and Westchester Counties and some areas of New York.
Community Services programs in Westchester and Rockland are located in Mt. Vernon Area; New Rochelle Services Area; Nyack Services Area; Peekskill Services Area; Yonkers Services Area; White Plains Services Area.
At each site, identified staff assist consumers with vocational service needs. RPC staff work collaboratively with other agencies and the business community to find suitable educational, training, and employment programs.

What is Staten Island Center for Independent Living, Inc.?
In Community Education we reach out to community resources and inform them of the needs and aspirations of the disabled in seeking independence. During Social Networking we offer accessible facilities for events. Our Advocacy Program promotes the rights of disabled consumers seeking entitlements. In our peer counseling we give the group and individual assistance, advisement and guidance in dealing with problems of Independence. Our services for the deaf fosters greater independence by expansion of communication skills, through a teletype system of relaying messages and sign language interpretation. What is the TECH Connection?
TECH Connection at Family Resources Associates, a nonprofit organization, empowers children and adults who are disabled, increasing options, independence and productivity through the use of computers.
Assistive Technology Specialist Joanne Castellano, Director of TECHConnection, provides a pleasant, non-commercial atmosphere wherein children and adults with disabilities receive encouraging guidance while gaining hands-on experience with alternative keyboards, switches, special input devices and communicative devices, and other forms of computer-related adapted equipment. Additionally, they acquire experience using a wide variety of computer software created to meet a broad spectrum of individuals needs, such as reading and writing challenges.
The center hosts workshops for professionals in education. Teachers from a tri-county central New Jersey area earn credit-hours learning how to better support students with needs that go beyond those of typical students.
At the center, or in a school, Joanne Castellano provides individual student technology evaluations. Her follow-up reports enable both the student's school as well as his/her family to provide the specific equipment and software that can most facilitate his educational progress.
TECHConnection maintains a lending library of software, and sponsors a computer re-cycling program. Donated computers are refurbished and made available to families financially challenged.

What is the TRI Business & Career Training Center?
TRI consists of two divisions, The Rehabilitation Institute (application enclosed) and TRI Business & Career Training Center. Both programs provide comprehensive vocational rehabilitation services for disabled individuals. TRI's major goal is to help the disabled become trained for competitive employment, become self-sufficient and achieve their personal goals.
TRI Business & Career Training Center located in Carle Place, New York has training programs in the following areas: Graphic arts/Desktop publishing, Office Technology which includes Filing, Mail Processing, Data Entry, Microsoft Word and Excel, Accounting clerk/Bookkeeping, Stock Inventory and Retail.
TRI also has a Transition to Work program for high school students that works in conjunction with their school districts.
We also provide Comprehensive Vocational Evaluations, Social Work and Placement services.
Many of TRI's consumers are sponsored by VESID (Vocational and Educational Services for Individuals with Disabilities)
All programs are individualized. Trainees work at their own pace aided by TRI's professional supervisors.
Job Readiness and Job Search Groups are ongoing and Internships in the business community are provided for consumers who need them.
Competitive employment is our goal for all trainees and we have achieved great success in this area.

United Cerebral Palsy Associations of New Jersey (UCPA/NJ) is known as an excellent resource in solving problems relating to education, employment, assistive technology, housing and advocacy for children and adults with temporary or permanent disabilities. This non-profit agency serves individuals with cerebral palsy, learning disabilities, traumatic brain injury, spinal cord injury, arthritis, spina bifida, drug or alcohol illness, mental retardation, stroke, hearing or visual impairments and others. What is Aspire?
Aspire has been serving individuals with a variety of disabilities for over 52 years. Aspire provides a wide range of services including employment, residential, therapy and medical, counseling, education and early intervention, social and recreational, day programs and service coordination. Specialized Support Services (SSS) is the vocational and supported employment component of Aspire. SSS works with individuals help identify and reach their vocational goals. Many people with diverse skills and challenges have found jobs in a wide variety of fields, from office and computer jobs, industrial and maintenance, to retail and hospitality. Services are highly individualized and include person-centered pre-employment exploration, job development, job coaching and follow-along services. Individualized non-traditional vocational assessments identify individual interests, requirements and the contributions they can bring to an employer. SSS also oversees the Community Employment Office, as Aspire is lead agency for this collaborative project of 27 agencies that work together to increase employment opportunities for individuals with disabilities in the community.

What is the United Cerebral Palsy Association of Westchester County?
Serves individuals with Cerebral Palsy, other neurological impairments, and developmental disabilities. Treatment in all programs may include therapeutic intervention, assistive technology, physical education, social services, vocational training and functional living skills.
The School serves children with multiple disabilities. Services include educational and therapeutic intervention emphasizing functional academics and independent skill development. The following services may also be provided as indicated on the students Individualized Education Plan; Speech Therapy, Physical Therapy, Occupational Therapy, Adapted Physical Education, Adapted Aquatics, Animal Facilitated Therapy, Vision services and Social Services.
The Jimmy Vejar Summer Day Camp serves children with multiple disabilities. Services include adaptive physical education, adapted aquatics, community integration, nature exploration and arts and crafts activities.
The Day Treatment Program for developmentally disabled adults offers a wide range of functional living skill training along with vocational, leisure, recreational services. Social, psychological and nursing services are also provided.
Vocational Services offers job readiness and vocational training for adults with developmental disabilities. Services include vocational evaluation and assessment, pre-vocational training, job placement, job placement, job coaching and follow-up services. Social services and nursing are also available.
Recreational and Leisure Services adults with developmental disabilities are offered through the Pioneer Club of UCP/W. This club offers a variety of recreational and leisure time activities including regular dinner dances, bowling tournaments and weekend retreats in the Catskills.
Assistive Technology services are offered through the Ariel and Evelyn Harris Assitive Technology Center. Some of the services offered through the library include:
- Access to a lending library of modified toys to provide children with recreation activities.
- Access to a lending library of adapted daily living devices to help individuals access their environments more easily.
- Use of a computer lab where consumers and their teachers can use applicable software to achieve educational and vocational goals, as well as, access to the Internet.
- Assessments and referrals for augmentative communication and daily living devices.
- Computer evaluations, adaptations, and training.
Case Management, Advocacy and Home Service is offered to individuals of all ages.
Residential Services for developmentally disabled adults are also provided. Referrals are through State and County offices only.

What is the Institute on Developmental Disabilities?
The Institute on Developmental Disabilities of the Graduate School of Public Health was authorized as a University Affiliated Program in 1991 under Public Law 101-496. The location of the PRUAP within the Graduate School of Public Health builds on the school's long tradition of networking with the community,agencies and programs, and on the interdisciplinary focus of its activities. The program is financially supported by the U.S. Department of Health and Human Services, Administration on Developmental Disabilities and the University of Puerto Rico.
The Institute fulfills its role by addressing the needs of individuals with developmental disabilities through interdisciplinary training, community-based servicing individuals with disabilities and their families in order to promote state-of-the-art experiences to students in the field. The Institute also develops alliances with the community, agencies and groups, educates professionals, paraprofessionals, consumers and families, provides technical assistance and disseminates information and research findings.

What is Vanderheyden Hall?
Vanderheyden Hall provides services to approximately 1500-2000
persons each year. Services include residential care in
homelike environments, educational services for our residents
and community, diagnostic services, crisis services, health
services, home and community-based care and a variety of customized
services designed to meet the needs of our population.
Vanderheyden Hall offers quality service by highly trained professional
staff for children who have generally been abused and/or neglected
and their families. Care is provided in a safe, nurturing environment
which allowed the child to work through issues and ultimately
realize a greater degree of potential.

What is Venture House, Inc. ?
The mission of Venture House is to enable consumers to lead successful lives in the community by providing opportunities for meaningful work, sustaining relationships, secure housing, adequate income, access to medical care, and opportunities to pursue educational and personal goals. Program activities center around work opportunities within the clubhouse as well as outside in the community. Participation is voluntary and members choose where they want to join in. Membership in the clubhouse not only provides a place to belong where members are needed, where their contributions make the program run.

What is VISIONS?
The agency was incorporated in 1926 as a not-for-profit 501(c) 3 organization. VISIONS programs include:
VISIONS @ Selis Manor, a new social service, recreation, computer training, volunteer program located at 135 West 23rd Street for tenants and community users who are blind.
Vacation Camp for the Blind (VCB), a 35 acre year-round facility in Rockland County, NY serving blind and visually impaired children, teens, adults, elders and families.
In-home Individual and Group Rehabilitation Training for people who are blind of all ages for Independent Living, Job Readiness and Employment.
Services for Blind Elders in Senior Centers and In-home Volunteers.
CIL Publications including Audio books For People Who Are Newly Blind and Textbooks with Lesson plans for Rehabilitation Teachers and Mobility Specialists.
VISIONS' multi-cultural and multi-lingual staff serves 1800 individuals annually in the Greater New York Metropolitan Area. All services are provided at no cost or for a suggested voluntarily contribution.
VISIONS is a member agency receiving support from the United Way of NYC and the NYS Rehabilitation Association, and is supported through a variety of sources including contributions, government grants, the five Borough Presidents, NYC and NYS elected officials. Donations to VISIONS are tax deductible to the full extent allowed by law. What is Warren Washington Assoc. for Mental Health?
Established in 1948, the Warren-Washington Association for Mental Health, Inc. is a not-for-profit provider of community mental and behavioral health services. Through the programs of AMH Behavioral Care, the Association assists children, adults, and families to develop stability and explore opportunities for growth. What is Westchester ARC?
Although Westchester ARC's services are varied, aimed at people ranging from infancy to post-retirement years, and delivered through facilities in every part of Westchester, all point to a broad-and achievable-objective: Help people with developmental disabilities become independent, productive and active in their communities to the fullest extent of their capabilities.

What is the Westchester Artificial Kidney Center?
Westchester Artificial Kidney Center is a non-profit outpatient hemodialysis unit serving the Westchester community since 1973. We are a 20 chair unit offering an interdisciplinary team approach to treating the dialysis patient. We have a high RN to patient ratio, an experienced and multilingual staff, a full-time, on-site Social Worker, consulting dieticians, and a travel coordinator. This comprehensive care is rendered on a personal level with consideration to the whole person. Our goal is complete patient rehabilitation. We are devoted to allocating funds to support research and education in kidney and related disease. Our mission statement reads, "The care of the patient is our reason for existence."

What is Wildwood Programs Inc.?
Wildwood Programs Inc. is a private, non-profit human service organization that provides an array of services to individuals with disabilities. Based on the mission of “working collaboratively with the community and families, Wildwood Programs empowers and enables children and adults with neurological impairments, learning disabilities, autism and other developmental disabilities to lead independent, productive and fulfilling lives”. Founded in 1967 by a group of parents, Wildwood has served several thousand individuals with disabilities in its extensive history.

What is the WNY Independent Living Center?
The WNY Independent Living Center assists people with disabilities. Services include finding housing, financial assistance, connections to vocational rehabilitation (VESID), transportation and everyday living skills. In addition, Independent Living Centers advocates for people with disabilities and partners with community services. Where services cannot be provided, Independent Living Centers will refer consumers to appropriate providers.

What is South Oaks Hospital?
SOH is fully accredited by the Joint Commission on Accreditation of Healthcare Organizations and is licensed by the New York State Office of Alcoholism and Substance Abuse Services. It is a member of the National Association of Psychiatric Health Systems, the New York State Association of Private Psychiatric Hospitals, and the New York State Providers of Alcohol and Substance Abuse Treatment. The Career and Educational Counseling Program is approved by the New York State Education Department and the Social Security Administration as a vendor of vocational rehabilitation services. As noted earlier, SOH has an extensive experience interfacing with both private and public organizations.
SOH's mission is to provide quality care with dignity and compassion to individual's and community. SOH provides education, advocacy, and treatment to support individual's in reaching their highest practicable levels of wellness and independence, while assisting as individual's safe timely return to work .
Our program is located one block from Nassau County transportation hub, two blocks from Suffolk County transportation hub and two blocks from the local train station, making our facility easily accessible via public transportation for clients who have no transportation.

What is YAI?
YAI offers one of the nation's most comprehensive networks of direct services. These encompass day and evening services, children's services; family support services; job training and job placement, residential services; recreation and camping; primary health care, specialty services, and certified home health care. Programs and services are provided in a variety of community settings and at home.
YAI Employment Initiatives Department was found in 1985 with the opening of its flagship program in Manhattan. Since that time, YAI's Employment Initiatives Department has expanded to nine supported work training programs in New York City, Westchester County and Long Island, serving over 200 individuals each year.
In an effort to provide even more opportunities for people with disabilities to lead productive lives, YAI has established an affirmative business, The General Store, in which people with disabilities work side by side with non-disabled individuals.
